Mom
and dad are living in their own home. I live an hour away and
have a full time job. Mom has been calling three or four times
a day and I am worried about my parents’ ability to manage
every day needs as they used to. I’d like an objective
opinion and some suggestions on how to address my concerns.
Dad
is in the hospital and can’t return home without a lot of help. The
hospital discharge planner is talking about placement or extensive
care at home. I don’t know the right thing to do.
Hospital
discharges often happen very quickly. Staff is overwhelmed
with large case loads and pressure to expedite discharges.
Discharges
home frequently use Medicare covered services to put together a “safe
discharge.” This is almost always a short-term solution (a couple
of weeks or a month) that may not begin until a week or so
after discharge. The long-term picture is what may really need
to be addressed.
The
need for a discharge to a facility (nursing home or “assisted
living”) can come as a surprise, especially if the recommendation
is for long-term placement. Making such an important decision
is life changing and should be handled thoughtfully. Adequate
attention to the specific needs of each patient and his/her situation
should be given.
We

is in a nursing home for rehabilitation. The discharge planner
is arranging plans for some care at home when she returns home. I
am concerned about who will help us coordinate services as the situation
changes over time once Mom is home.

can help families in understanding and dealing with care for a loved
one with dementia and other debilitating conditions. We can
evaluate functional levels and make suggestions for care management.

often receive referrals from physicians, attorneys or other professionals
with elderly clients who need care management or assessments. We
work along with other professionals involved in care to provide a comprehensive team approach. Referrals to other
professionals with expertise in various specific areas of service
are made as needed.
